Supportive Outreach

 

     Laurel Hill Center’s Supportive Outreach Team works with individuals identified through the Lane County Frequent User Service Engagement (FUSE) project.  Individuals in FUSE are unhoused and have been high utilizers of hospitals, jail, and courts and are often facing significant challenges with medical, mental health, and substance use disorders.

     The Supportive Outreach Team offers street outreach with a focus on establishing rapport and trust with people who have historically been reluctant to participate in services.  Participants are provided with supplies to support their basic needs, case management, housing assistance, peer support, and assessment services to help connect them with permanent service providers.  Individuals will receive a “Front Door Assessment” to access housing through Lane County’s Coordinated Entry system.

     As part of a community-wide collaboration, the team works alongside other agencies, law enforcement, health care providers, and other community members to form a partnership to help individuals address problems that are often complex and chronic in order to manage health needs, provide support with behavioral challenges, and successfully transition to stable housing.
